Cerberos Brokers Pty Ltd are the appointed licensed general insurance brokers for the CFMEU Mining & Energy QLD District Insurance Goonyella Riverside Scheme. Cerberos administer the Income Protection (Weekly Injury & Sickness Benefit).
The Life cover component is handled by Paul Bonney (ARN 297779), a Representative of Cable House Pty Ltd (ABN 78 139 055) and an authorised Representative of Cable House Financial Planning Pty Ltd (ABN 42 640 473 445) AFSL 522916. Cerberos is not licensed to transact life insurance and cannot offer any advice on the MetLife cover component of the scheme.

How To Purchase
- To join the scheme simply select the APPLY ONLINE button and follow the prompts (please use your surname as the reference). Your cover will commence from the date of your first deduction.
- Should you wish to disclose any pre-existing condition to the underwriters prior to commencing cover, please complete the attached Ezi Debit form, together with the Declaration of Health form and return to our office by email info@cerberos.com.au or post- PO Box 1305 Spring Hill QLD 4004.

A requirement of joining the life cover is that all new members MUST COMPLETE THE GROUP LIFE INDIVIDUAL APPLICATION FORM. No cover available until insurer confirms acceptance. Please note cover for Income Protection can be commenced immediately and deductions are amended as and when Life Cover is finalised.
